Fatal Crash Involving Jefferson Transit Bus on SR-104

PORT TOWNSEND — One person is dead and several others — including two Sequim residents — were injured in a crash involving a Jefferson Transit bus Tuesday evening in Jefferson County.

According to the Washington State Patrol, the three-vehicle collision happened just before 7 p.m. on State Route 104 near milepost 9.

Troopers say a westbound vehicle crossed the center line and struck an eastbound Jefferson Transit bus head-on. A second westbound vehicle traveling behind it then collided with the bus as well.

The driver who crossed the center line — identified as 17-year-old A. Pelilu Wines of Poulsbo — died at the scene.

Two people from Sequim were also injured in the crash. Troopers say 41-year-old Christopher Fiorini of Sequim, who was driving the third vehicle, and 68-year-old Elizabeth Fiorini of Sequim, a passenger, were both transported to Jefferson Healthcare in Port Townsend.

The driver of the Jefferson Transit bus, 62-year-old Michael Berrett of Port Townsend, was seriously injured and airlifted to Harborview Medical Center in Seattle.

A passenger in that vehicle was not injured.

Three people were hurt overall in the crash, and the cause remains under investigation. State Patrol says it has not yet determined whether drugs or alcohol were factors.
